THE CHAMPLAIN CVD PREVENTION STRATEGY


Catalyzed by the University of Ottawa Heart Institute, health and community partners from across the Champlain region came together to develop the Champlain CVD Prevention Strategy. This five-year plan is designed to significantly reduce the burden of CVD in the Champlain District of Ontario through evidence-based, best practice approaches to disease prevention and health promotion.

The Champlain CVD Prevention Network (CCPN) was formed to provide leadership in implementing the Champlain CVD Prevention Strategy. This represents the first time leaders in primary and specialty care, public health, community health, and research settings have come together with a common vision for CVD prevention.
