OUR GOALS

The CCPN is poised to provide "state-of-the-art" interventions in six key areas: primary care, specialty care, hospitals, schools, workplaces, and communities.

Phase I of the Champlain CVD Prevention Strategy (2006-08) identifies six large-scale, population-level priority initiatives. These initiatives were identified by the CCPN Expert Panels and endorsed by the CCPN Coordinating Committee as the most important actions to improve the CVD health of Champlain residents.

• Champlain Healthy School-aged Children Initiative
• Champlain Hospital-based Smoking Cessation Network
  • Expansion of evidence-based smoking cessation interventions in hospitals across Canada (limited access)
• Champlain Get with the Guidelines Initiative - Acute Coronary Syndrome
• Improved Delivery of Cardiovascular Care (IDOCC) through Outreach Facilitation (Primary Care)
• Champlain Sentinel CVD Surveillance Program - Champlain Community Heart Health Survey

A strategy renewal phase is planned for 2009 to identify gaps and new priority areas for action in 2009-11.
