PERFORMANCE MANAGEMENT




The CCPN is committed to demonstrating results for Champlain residents, the Champlain Local Health Integration Network (LHIN), and the Government of Ontario.

The Champlain CVD Prevention Strategy includes a robust performance management system to track the progress of each of the six CCPN priority initiatives as well as the impact of our collective work on health outcomes in the Champlain District.

The CCPN performance management plan has been designed to align activities and outcomes to Ontario's Health System Strategy. Performance indicators (process, outcome, and impact) and annual benchmarks have been established for each priority initiative.
