Champlain Local Health Integration Network
Local Health Integration Networks (LHINs) are non-profit organizations designed to plan, coordinate and fund the delivery of health care services across the province of Ontario. The Champlain LHIN, one of 14 LHINs in the province, encompasses the City of Ottawa, Renfrew County, the Eastern Counties of Stormont, Dundas & Glengarry and Prescott & Russell, as well as the northern parts of Leeds, Grenville & Lanark.
 
Champlain Regional Stroke Program
The Champlain Regional Stroke Centre is one of nine regional stroke centres designated as part of the Ontario Stroke Strategy. Based out of the Ottawa Hospital, Civic Campus, the Centre provides leadership, development, implementation and integration of stroke care throughout the region and across all points in the spectrum of stroke care. This includes promotion, clinical and secondary prevention, acute care, rehabilitation, and home care.
 
Department of Family Medicine, University of Ottawa
The Department of Family Medicine at the University of Ottawa has a long and strong tradition in the areas of clinical care, education and research. Members of the Department have particular interests in care of the elderly, prevention, collaboration between nurses and doctors, informatics, immigrant health, chronic disease management, and knowledge translation.
 
Eastern Ontario Community Primary Health Care Network
The Eastern Ontario Community Primary Health Care Network includes community health centres (CHCs) from across the Champlain LHIN. Collectively, the Network serves urban, suburban, rural, francophone, and urban aboriginal communities in Eastern Ontario and meets to share information, discuss joint program planning and delivery, and collaborate on advocacy issues. Individually, the CHCs are community-governed agencies that offer primary health care and social services based on the social determinants of health and the principles of health promotion and education.

Eastern Ontario Health Unit
The mission of the Eastern Ontario Health Unit (EOHU) is to promote and protect the health and well-being of its citizens by preventing diseases and accidents, and advocating for a safe and healthy environment and healthy public policies. The EOHU serves residents of the counties of Stormont, Dundas, Glengarry, Prescott, Russell and the City of Cornwall.
 
Élisabeth Bruyère Research Institute
The Élisabeth Bruyère Research Institute, a partnership of the SCO Health Service and the University of Ottawa, supports researchers who contribute to the relevant and practical knowledge needed to improve continuing care; specifically, to foster quality of life for the elderly, and to improve Primary Care, Rehabilitation, and Palliative Care. The Institute works closely with community and long-term care partners to bring faculty into service delivery, students into service learning, and service providers into research and education.
 
Healthy Active Living & Obesity Research Group, CHEO
The Healthy Active Living & Obesity (HALO) Research Group is housed within the Children's Hospital of Eastern Ontario (CHEO) Research Institute. HALO's vision is to provide national leadership and research excellence in healthy active living for the prevention and treatment of obesity in children and youth.
 
Heart and Stroke Foundation of Ontario
The Heart and Stroke Foundation of Ontario is a community-based volunteer organization whose mission is to reduce the risk of premature death and disability from heart disease and stroke by raising funds for research and health promotion.
 
Institute of Population Health, University of Ottawa
The Institute of Population Health (IPH) is a consortium of 10 faculties of the University of Ottawa: Arts, Education, Graduate and Postdoctoral Studies, Health Sciences, Law, Science, Social Sciences, Medicine, Management and Engineering. As a consortium, the IPH coordinates, initiates, and supports interfaculty and multiparty, transdisciplinary research and training in Population Health.
 
Leeds, Grenville & Lanark District Health Unit
With its main office in Brockville, the Leeds, Grenville & Lanark District Health Unit offers a wide range of services to promote healthy living, healthy growth and development, prevent illness and injury, and control communicable diseases in the community. Services are available to individuals and groups of all ages in a variety of locations.
 
Ottawa Public Health
Ottawa Public Health (OPH) is responsible for delivery of public health services including health protection, disease and injury prevention, control of communicable diseases, and family health services. OPH offers bilingual programs and services to Ottawa residents in both rural and urban communities, spanning Cumberland in the east to Fitzroy Harbour in the west, and from the Ottawa River in the north to Osgoode in the south.
 
Renfrew County & District Health Unit
With its mandate of "promoting healthy people in a healthy environment", the Renfrew County & District Health Unit (RCDHU) covers a large geographic area that includes the County of Renfrew, the City of Pembroke, the Township of South Algonquin in the District of Nipissing and most of Algonquin Provincial Park. The RCDHU's main office is located in Pembroke, with branch offices in Arnprior, Barry's Bay, Deep River and Renfrew.
 
The Ottawa Hospital
The Ottawa Hospital (TOH) is a leading academic health sciences centre in Canada. Together with its partners, TOH is becoming nationally recognized for outstanding patient care, education and research. TOH is a compassionate provider of comprehensive, high-quality, patient-focused health care services in English and French to a population of 1.5 million people in Ottawa and Eastern Ontario. It boasts specialty centres in cancer, heart, kidney and vision care, as well as rehabilitation services.
 
University of Ottawa Heart Institute
The University of Ottawa Heart Institute (UOHI) is Canada's largest and foremost cardiovascular health centre dedicated to understanding, treating and preventing heart disease. We deliver high-tech patient care with a personal touch, shape the way cardiovascular medicine is practiced, and revolutionize cardiac treatment and understanding. We build knowledge through research and translate discoveries into advanced care. We serve the local, national and international community and are pioneering a new era in heart health.
