Overview
The Champlain Get with the Guidelines Initiative is a hospital-based quality improvement initiative designed to create a regionalized approach for the secondary prevention of cardiovascular disease (CVD) according to evidence-based practice guidelines in patients admitted to hospital with Acute Coronary Syndrome (ACS) or Heart Failure (HF).
The Champlain Get with the Guidelines program has created a network of hospitals in the Champlain District which:
- Ensure all providers are trained in the evidence-based secondary prevention treatments;
- Ensure all patients admitted to hospital receive life-saving evidence-based secondary prevention treatments (lifestyle and medications);
- Empower patients to be more involved with their evidence-based care;
- Continuously measure outcomes and improve the quality of care in our region; and,
- Improve patient outcomes.

The regional rollout of the Get with the Guidelines Initiative is being directed by a team at the University of Ottawa Heart Institute (UOHI) who spearheaded the creation of the Canadian adaptation of the American Heart Association Get with the Guidelines tools and support its dissemination to partner hospitals committed to excellence in patient care.
The Champlain Get with the Guidelines Initiative is a Priority Initiative of the Champlain Cardiovascular Disease Prevention Network (CCPN). The CCPN’s overarching goal is to prevent 10,000 premature deaths from heart disease and stroke by 2020. |
