Information from this survey will help us better understand our community’s health needs. If you are 35 to 70 years of age, and plan to live in the same residence for the next 5 years, you may be eligible to participate. To find out more, visit this portion of our web site.

The Champlain Community Heart Health Study (CCHHS) is a community-based survey designed to collect key information relevant to the heart health of the residents of the Champlain District. Participation will provide valuable information that will help us evaluate the extent of health problems associated with major health concerns such as heart disease, high blood pressure, diabetes and obesity in the communities of the Champlain District.

The CCHHS is one of six Priority Initiatives of the CCPN.
